PPE Requirement

The following is an acknowledgement of the PPE requirement on this load. You must have all the items listed below. You also must confirm that you will have them on when you are on the job site at pickup or delivery.

  • Full PPE
  • Shirt with Sleeves
  • Long Pants
  • Safety Vest
  • Steel Toed Boots
  • Safety Glasses
  • Hard Hat

I agree to wear all the safety equipment listed above at the required loading and/or unloading locations. I understand there will be a $1,000 reduction if I do not comply with all requirements.
